Important Physical Information/Dates Released.

Annual Wrestler physicals will be held on:
Saturday- April 23rd or Saturday May 7th from 8:30am-12:00pm at Old Town Family Practice (Dr. Swaldi’s office)
413 W. Bethel Road, Suite 300

There are 6 required forms:
1.       Physical Cover Letter
2.       Preparticipation Physical Form
3.       Emergency Information Form-Must be Notorized!
4.       UIL & CISD Athletic Policies
5.       UIL Acknowledgement of Rules form
6.       UIL Steroid Agreement

Athletes should have all forms filled out, signed, notarized, etc. before the first day of their practice.

Secure photo albums of Coppell Wrestlers are now accessible through the Coppell Wrestling Booster Club website. Refer to the "Photos" tab on the home page, then click "Shutterfly Photo Albums" and you will be linked directly to the private site for login and password information. The one-time registration process is quick and easy.
The Coppell Wrestling Booster Club is classified as a non-profit 501c-3 organization. The photo gallery is available free of charge for the benefit of our members.
